Change in Passport Renewal Applications effective May 1st, 2016


DS 82 ONLY


Passport Renewals


DUE TO THE HIGH VOLUME OF PASSPORT APPLICATIONS VERNON TOWNSHIP WILL NO LONGER ACCEPT ADULT RENEWAL APPLICATIONS BEGINNING MAY 1, 2016


To renew an adult  passport, visit www.travel.state.gov and download form DS-82 along with the instructions.


YOU CAN RENEW BY MAIL IF ALL THE FOLLOWING ARE TRUE

  1. *    Your Most Recent U.S. Passport:

  2. *    Is undamaged and can be submitted with your application;

  3. *    Was issued when you were age 16 or older;

  4. *    Was issued within the last 15 years; (not expired more than 5 years) and

  5. *    Was issued in your current name or you can legally document your name change with a                                 certified copy of your marriage certificate or government-issued document evidencing your legal name change.


Mail the renewal application yourself- You do not need to go to an acceptance facility!

All minors age 15 and under must apply in person for a first time passport using form DS-11.  Minors and young adults with a currently expired or expiring 5 year passport must also apply in person using the DS-11 form.
